Schedule an Appointment
To schedule an appointment, please contact a doctor’s office listed. New patients will be asked to complete a standard new patient registration form and medical history form, both of which can be reviewed under patient forms.
Insurance Authorization
Please be advised that it is the patient’s responsibility to obtain the necessary authorizations from their insurance company prior to seeing a doctor at Beth Israel Orthopaedics and Sports Medicine. These authorizations vary with each insurance company, and we advise that you consult your insurance company or your primary care provider to determine if any pre-authorization or doctor referrals are required. If your insurance company requires an authorization, this authorization form must be provided at the time of your visit to the doctor.
Payments and Deductibles
All payments for office visits, if required by your medical insurance company, are due on the day of your appointment with the doctor. Any co-payments required by your insurance plan will be expected to be paid on the day of service, and for patients seeing our doctors out-of-network, the full office visit fee is payable at the time of the appointment.
Beth Israel Orthopaedics and Sports Medicine will submit the routine bills for office visits directly to your insurance company for all in-network patients and will also extend the courtesy to submit bills to the insurance company for those patients who see our surgeons out-of-network. It is the office policy that the patient will fulfill the obligation of any deductibles required by their insurance provider.
Payment Method
Payments required on the day of your office visit may be made by cash, check, or credit card.
Billing Questions
Any questions regarding your medical bills should be directed to the office manager in your respective surgeon’s office.
Insurance Plan Reception by Surgeon
At least one of our surgeons participates in nearly all the insurance plans offered in the New York metropolitan area. Many of our surgeons are in-network and others are out-of –network providers.
Please speak directly with the individual office staff to review your options. Insurance plans offer various opportunities both in and out of network, and our experienced office staff can answer any questions.
